By clicking on the Menu -> Create -> Check Wizard or by clicking the icon once you have chosen where to place the check-wizard (which can be of one or more check-boxes with their relative labels) a pop-up window will appear with different options:

It allows you to design a check-box system almost automatically by following the options desired. You must compile the entire Wizard (made up of 3 steps) before seeing the result on the screen.

1st Step

It will ask you if you want more then one check-box and what kind of response you want for every check-box. In order to facilitate your task, at the bottom of the wizard there will be an example of what should show up depending on the choice. Click next to go to the following step.

2nd Step

Depending on your choices, you will be given different types of spaces to fill in.
If you are satisfied with your choice click Next otherwise click Previous to go back to make changes from the beginning.

3rd Step

The Results of your 1st and 2nd Step choices will appear and you will be asked to confirm that you made the right choices. Click on Create, or to go back, click on Previous.

Remember you can always select the grouping function by leaving the mouse pressed on the form template to include all the fields that you want to move. See also selecting, moving or resizing fields.
