By clicking into the Menu -> Create -> Check Box or by clicking the icon you can design and define on the form template:
width and height by dragging the mouse to the size you want.

To move or resize the check-box see: Selecting, Moving or Resizing Fields .
It can be positioned anywhere in the form and once placed it can be moved or resized.

Options related to the check-box:

Value: You can choose between various types of answers when the check box is checked.
Yes/No means that if it is checked the answer is Yes if not it is No.
Yes/None means that if it is checked the answer is Yes otherwise Nothing has been answered and is left blank.
No/None means that if it is checked the answer is No otherwise Nothing has been answered and is left blank.
Other: means that you can set a customized response like Blue/Red etc.. depending on the question asked.
